Here's how the program works:

FAAN members can nominate their schools (elementary, intermediate, and high schools) to receive a free copy of the School Food Allergy Program.

Here's how to nominate your school:

1. Enter your information in the form below.
2. Click on “Submit.”
3. Print out a hard copy.
4. Take the hard copy to your school, and get a school representative or principal to complete the online request process.

Shipments will continue until supplies are exhausted.

Please use one form per school.
